25 YEARS OF ADHESIVE PARTIAL RESTORATIONS FOR ENDO-TREATED POSTERIOR TEETH

A large number of devitalized teeth are lost not so much as a result of poor endodontic therapy but because of fractures resulting from inappropriate restorative treatment. Although it is true that a lot of attention must be given to designing the endodontic access so as to spare the healthy tooth structure and ensure its future resistance, it is equally correct to say that it is often difficult if not impossible to do this in practice. Everybody knows that in everyday clinical practice it is sometime necessary to sacrifice the crown more than originally envisaged “in the design” because all this is with a view to achieving the objective of modern endodontic practice: cleaning and filling the root canal system in three dimensions.
Ultra-conservative opening for pulp access, so called ninja-access and super minimal cavity designs have to be considered more as a theorethical approach than a real possibility for the clinicians because all of us are much more often faced with destroyed teeth than to healthy teeth where to design such microscopic openings!!!
The world literature shows how clinical success is linked more with the restorative choice than with the amount of coronal structure preserved during a given treatment and in the last 20 years a very particular interest has been developed in obtaining a perfect “coronal” seal being it the essential to guarantee a successfull “apical” seal; the interest shown in this field can be seen from the number of papers written on this topic.
The real problem is how and which material should be used to obtain a conservative restoration of an endodontically treated teeth. Really different is the clinical approach to treating the teeth of anterior arch when related to that one used for latero-posterior teeth.
Nowaday we can safely use direct and indirect conservative adhesive approach for the majority of the anterior cases and, looking at the last results presented in the international literature, much more than yesterday we can restore endodontically treated posterior teeth using adhesive indirect techniques.
The aim of this lecture will be:
- to present concepts for post-endo indirect restorations but also,
- trics to transfer principles for vital teeth treatments to not-vital ones.
