The introduction of the surgical operating microscope has fundamentally changed the procedures. The author firmly believes that endodontic surgery is not an APICOECTOMIE, we can now locate and treat second and third molars and manage procedural errors such as overfilling inside mandibular canal, or sinusitis due to dental infection by opening a window through the bone at the apical area. However, another way of thinking will be presented hereinafter, especially as regards the posterior sites. Endodontic microsurgery treatment could failed sometimes for many reasons and at different situations: serious anatomical elements, over bleeding when hemostasis is the most important steps, deep bone cavity where the apex is deeply located, thus, a new bone trephination technique will be described step by step in this presentation, for bone trephination is the most important and delicate stage allowing us to locate the apex and avoid post-operative problems. In this presentation we will illustrate the most difficult and complicate situations or clinical cases in microsurgery using digital pictures, video clips and slides so that the participants will have a clear understanding of surgical procedures and principles.
